Alpha arbutin vs hydroquinone in Pakistan: they are not competitors

Both slow the enzyme that produces melanin, which is why they get compared. The difference is not strength alone, it is duration. Hydroquinone is stronger, works faster, and is designed for a short supervised course with a defined stop date. Alpha arbutin is gentler, works slowly, and is safe to use for as long as you like. Treating them as rivals is how people end up on hydroquinone for two years.

The comparison, on the things that matter

Hydroquinone works in six to twelve weeks and should stop at around three to four months. Alpha arbutin takes eight to twelve weeks to show anything and can run indefinitely. Choose by how long you need to be on something, not by which is stronger.

What each one costs in Pakistan

Alpha arbutin at a stated 2% costs between Rs. 1,449 and Rs. 3,850 here. Hydroquinone is cheaper per tube and is a prescription decision with a fixed course length, which is the real difference in price between them.

Prices were read from each brand's own online store on 11 September 2026. Imported products have no Pakistani store, so those rows are a reseller's listing read the same day and are labelled as such. Where a seller does not state a size or an active's percentage, that is recorded as not stated rather than guessed at — the absence is the useful part, because you cannot compare a dose that nobody publishes. Why hydroquinone has a stop date

Long uninterrupted use risks exogenous ochronosis — a blue-grey discolouration that is far harder to treat than the pigmentation it was used for, and disproportionately reported in deeply pigmented skin. That is the entire reason the course is time-limited, and the reason unlabelled mix creams are dangerous: you cannot time-limit what you do not know you are taking.

The Pakistani market problem

Hydroquinone is not the problem; unlabelled hydroquinone is. A large share of the mix and fairness creams sold here contain it alongside a steroid and a retinoid, with no percentage printed, no stop date given and no supervision. That combination is what produces the rebound and the thinned skin people arrive at a dermatologist with.

A cream with hydroquinone printed on it, at a stated percentage, used for three months under supervision, is a legitimate medical treatment. A cream with no ingredient list that works in ten days is not the same thing, whatever the shop says is in it.

Where alpha arbutin fits

As the long-term option, and as maintenance after a hydroquinone course ends — which is when pigmentation most often returns. It is also the sensible first choice for anyone who has not seen a dermatologist yet, because it cannot be overdone in the way the stronger option can.

What both need in order to work at all

Daily broad-spectrum sunscreen. Both slow pigment production; neither stops UV from triggering more. Without SPF, both are running against a tap left open, and that is the usual explanation for a treatment that seemed to do nothing.

What else is worth knowing about

Azelaic acid, tranexamic acid, niacinamide, cysteamine and retinoids all have evidence in pigmentation, and several are safer than hydroquinone for long use. For melasma in particular, a dermatologist will usually build from several of these rather than pick one.

Which to choose, plainly

If pigmentation is mild and you want something you can run for a year, alpha arbutin with sunscreen. If it is stubborn or extensive, see a dermatologist and ask about a supervised hydroquinone course with a written stop date, then move to arbutin for maintenance. Do not choose an unlabelled cream in either case.
